Overview

The Senior, Youth and Behavioral Congregate Settings Team  at the Chicago Department of Public Health is responsible for the COVID-19 response within specific congregate living facilities around Chicago. Facilities that fall under their authority include, but are not limited to,  the following facility types: Assisted Living, Supportive Living, Senior Independent Living, Behavioral Health Facilities (including SUPRS, Residential Treatment Facilities, Behavioral Health) and Youth Homes.

The Core Principles of COVID-19 Infection Prevention and Control

  1. Vaccination
  2. Source control (masks, face coverings, and other respiratory protection)
  3. Resident and staff testing
  4. Hand hygiene (use of alcohol-based hand rub is preferred)
  5. Physical distancing
  6. Appropriate use of personal protective equipment (PPE)
  7. Instructional signage throughout the facility and communication
  8. Infection prevention and control education and competency
  9. Cleaning and disinfecting high frequency touched surfaces and equipment
  10. Appropriate ventilation and engineering controls to improve air quality
  11. Effective cohorting

Needs Assessment

 The Chicago Department of Public Health (CDPH) is conducting this survey in order to identify areas of collaboration and gather information on future needs of your facility.

This information will assist us in identifying  support strategies that best meet your needs.  The information you provide will be used solely for capacity building and will not be shared wih others outside CDPH.  This survey will take approximately 15 minutes to complete.  We ask that your facility only completes one survey at this time.

Reporting Information


Lab Confirmed testing
Lab-confirmed cases of COVID-19 are reportable conditions to the Chicago Department of Public Health. Facilities should Report lab-confirmed resident and staff cases within 24 hours to CDPH using a line list template. The line list will then be uploaded here. For a copy of the line list template, please contact the Senior, Youth, & Behavioral Congregate Living Settings Team at SYBCOVID@cityofchicago.org

Vaccination and testing Data:
Facilities that are not required to report COVID-19 aggregate vaccination and testing data into the National Healthcare Safety Network (NHSN) shall report this data to IDPH weekly utilizing the online form at LTC Weekly Reporting COVID-19 Vaccinations and Testing
The required information matches that submitted by CMS-certified facilities to NHSN.

Rapid Antigen testing:
Facilities testing with rapid Antigen tests must report positive test results via Simple Report or via Redcap
