The Healthcare Settings team at the Chicago Department of Public Health is a committed partner of the Making Dialysis Safer for Patients Coalition. Our goal as a committed partner organization is to improve adherence to CDC Core Interventions and Guidelines to reduce bloodstream infections among hemodialysis patients and engage dialysis facilities, patients undergoing hemodialysis, and other partners to reduce infections.
As a committed Coalition partner we will conduct activities to: increase awareness among our organizational members through communication and educational efforts about CDC Core Interventions and Guidelines to reduce bloodstream infections; implement CDC Core Interventions by facilitating adoption through use of the Making Dialysis Safer materials and resources; and share experiences and findings by providing a summary of our activities and metrics, sharing implementation strategies, and collaborating with other Coalition participants.

The Centers for Medicare & Medicaid Services (CMS) Survey and Certification Program certifies ESRD facilities for inclusion in the Medicare Program by validating that the care and services of each facility meet specified safety and quality standards, called "Conditions for Coverage." The CMS Dialysis page provides basic information related to survey and certification of dialysis facilities for ESRD surveyors and dialysis providers. It includes links to applicable laws and regulations and provides resources to support and assess compliance with Federal regulations.

Infection Control Assessment and Response Tools (ICARs) (English & Spanish)
Purpose: ICAR tools are used to systematically assess a healthcare facility’s IPC practices and guide quality improvement activities (e.g., by addressing identified gaps).
How CDPH can help: The ACHOO team is engaged in comprehensive visits to dialysis clinics throughout Chicago, during which they perform a specific ICAR developed for Hemodialysis Facilities. Upon concluding an ICAR, the team provides post-report recommendations, equipping these facilities to proactively address any regulatory inspections. It is essential to emphasize that their visits are inherently non-regulatory and non-punitive. Instead, the primary objective is to foster collaboration among departmental teams within each facility and identify areas for enhancement.
