- Healthy students are better learners, and academic achievement bears a lifetime of benefits for health. Schools play an important role in promoting the health and safety of children and adolescents by helping them to establish lifelong health patterns.
- The Whole School Whole Community Whole Child (WSCC) model is a CDC framework for addressing health in schools. The WSCC model is student-centered and emphasizes the role of the community in supporting the school and the importance of evidence-based policies and practices.
- The WSCC model has 10 components:
- Physical education and physical activity.
- Nutrition environment and services.
- Health education.
- Social and emotional climate.
- Physical environment.
- Health services.
- Counseling, psychological and social services.
- Employee wellness.
- Community involvement.
- Family engagement.
- This page will outline the CDPH programs that support the component of health services in schools.

Immunizations
- SUMMARY OF PROGRAM
- Vaccines can prevent many harmful and deadly diseases in infants, children, teens, adults and travelers of all ages. Getting vaccinated is one of the safest and most effective ways for you to protect your health. Over the years, vaccines have saved and improved millions of lives.
- Many vaccines are also offered at your primary care office or at pharmacies on a walk-in basis. Use the Health Map Vaccine Finder to find a location that offers vaccines.
- The Chicago CareVan is a longstanding partnership between the Chicago Department of Public Health (CDPH) and Blue Cross Blue Shield of Illinois (BCBS). The CareVan is a mobile immunization clinic that offers services at no out-of-pocket cost at Head Start, childcare centers, schools, adult high-risk centers, WIC centers, and community events. The CareVan provides all immunizations that are recommended by the Centers for Disease Control and Prevention (CDC), except for Varicella (chickenpox) vaccine.

- WHO CAN ACCESS THE PROGRAM:
- CDPH operates two immunization clinics and one mobile clinic (CareVan) where children and young adults aged 18 years and under can get required and recommended vaccinations and seasonal flu shots at no cost. Chicago residents who are 19 years and older and are uninsured can receive some recommended vaccines. During Flu season, ALL Chicago residents are eligible to receive Flu vaccine, regardless of insurance coverage.
- Appointments are highly recommended. Walk-ins are accepted. To schedule an appointment an immunization clinic or CareVan, please visit getvaxchi.chicago.gov/
- At your appointment, please bring an up-to-date immunization record for the clinic staff, if available. If you have insurance, please bring your insurance card. - How to Locate Immunization Records
- You can track down your vaccination history via the IDPH self-service portal Vax Verify or by requesting your vaccination history by filling out this form.
- If you don’t have a copy of your childhood vaccination records at home, call your healthcare provider or medical clinic where you received a vaccination shot. You can also check with your high school/college or previous employers that may have required immunizations. Former CPS students can request immunization records by submitting an online application. Finally, you can call the CDPH Immunization Program directly at 312.746.6129 to submit a request to check the state immunization registry I-CARE.
- If you can’t find your personal records or records from the doctor, you may need to get some of the vaccines again. While this is not ideal, it is safe to repeat vaccines. The doctor can sometimes do blood tests (titer) to see if you are immune to certain vaccine-preventable diseases also.
- COVID-19 Missing Dose Information
- Please complete this form to report any missing COVID-19 vaccinations.
- If your COVID-19 vaccination was administered in the city of Chicago, you will need to reach out to the Health Care Professional or Clinic Site where you were vaccinated to request that they report the dose to the immunization registry.
- If you received your 1st, 2nd and/or booster doses at different locations we may need to ensure that they've been properly logged, and your records have been linked. It's also possible that there's been a data entry error or that there's been a delay in entering the information for one or both doses due to the high volume of entries. The purpose of this form is to ensure that your vaccination record is as complete and accurate as possible, and we will update your record as we are able.

Oral Health
- School Based Oral Health (SBOH)
- SUMMARY OF PROGRAM
- CDPH School-Based Dental Program collaborates with CPS to provide oversight for the oral health programs in K through 12th grades in the Chicago Public Schools.
- Dental professionals provide dental exams/screenings, oral prophylaxes (cleanings), fluoride varnish treatments and dental sealants as needed to students in the Chicago Public Schools.
- Students provide a signed consent form to participate in the program.
- WHO CAN ACCESS THE PROGRAM:
- All schools in Chicago Public Schools K through 12th grades

Sexual Health
- CHAT
- SUMMARY OF PROGRAM
- CDPH, in partnership with Chicago Public Schools and Planned Parenthood of Illinois, provides sexual health education and STI testing to high schools across the city. This once-yearly program includes a presentation on sexual health, optional testing for chlamydia and gonorrhea, a confidential session with a health educator, and access to care services.

- CONDOMS 4 SCHOOLS

- CDPH has historically provided condoms to schools upon request, but schools this was left up to individual principal discretion. Effective school year 2021-2022, Chicago Public Schools amended their Sexual Health Education Policy to mandate that every school serving grades 5 and up must maintain a condom availability program. CDPH provides condoms upon request to schools at no cost (including charter, contract, and options schools, which are not required to follow this policy). CDPH also provides condoms to other embedded organizations serving schools, such as school-based health centers.

Vision Health
- SUMMARY OF PROGRAM
- The Chicago School-Based Vision Program (SBVP) is a collaboration led by the Chicago Department of Public Health (CDPH) with Chicago Public Schools (CPS) to provide vision health services to Chicago children.
- SBVP offers vision services directly on site to students at CPS schools.
- The program provides high quality comprehensive vision exams, prescription eyeglasses, eye health education, referrals and follow up for eye care at no cost to all CPS students.
